You can mail your completed application and check payment (Please make checks payable to: "Streamwood Summer Celebration") to:

If you would like to pay online, please click here; you will be directed to the Village of Streamwood’s Self Service website. Then, click “SC Food Vendor Payment.” All major credit cards are accepted (Visa, MasterCard, Discover and American Express). There are no additional fees for paying online.
